Continuing Care/Relapse Prevention
Description
The continuing care/relapse prevention program concentrates on emotional and psychological growth, development of positive peer relationships, problem-solving skills, fourth and fifth step work, and obtaining a sponsor. Resources required regular attendance in self-help groups. Continuing care/relapse programming is recommended for persons having completed a primary treatment program. The counseling staff will help facilitate the client's ability to remain sober through developing a personal recovery program. Staying Sober, A Guide for Relapse Prevention, published by Hazelden, is used as a guide to build a recovery program. Group therapy, videos, and discussion will be a part of the process. The client is held accountable for demonstrating change. The client is encouraged and supported in specific behaviors that facilitate positive change.
Group sessions are 1 ½ hours in length, one day per week for a periods of approximately 8 to 12 weeks. Wisconsin Uniform Placement criteria will be used to determine placement.
Program Goals:
The Client will:
- Maintain sobriety.
- Demonstrate use of outside support systems and need of a personal recovery program to assist in long term recovery.
- Demonstrate personal strategies that promote healthy life changes.
- Complete Commitment to Sobriety Workbook.
- Verbalize personal relapse signs.
- Demonstrate how he/she deals with relapse warning signs.
- Develop an understanding of the cross-addiction concept.
